Central motor conduction time may predict response to fampridine in patients with multiple sclerosis

چکیده
INTRODUCTION Most patients with multiple sclerosis (MS) experience walking impairment during the course of their disease. From an MS patient’s perspective, walking ability is the most important bodily function. Currently, sustained-release oral fampridine (fampridine-SR) is the only drug approved for the symptomatic treatment of walking disability due to MS. Fampridine is a potassium channel blocker that improves the impaired axonal conduction associated with CNS demyelination. However, only a subpopulation of MS patients with walking disability respond to fampridine. Herein, we prospectively evaluated whether pretherapy motor-evoked potentials (MEP), reflecting pyramidal demyelination by an increased central motor conduction time (CMCT), are suited as a predictive therapeutic response marker.

Central motor conduction time in progressive multiple sclerosis. Correlations with MRI and disease activity.
The purpose of this study was to relate abnormalities of motor conduction time to the presence of spinal cord MRI lesions in progressive multiple sclerosis and to investigate the relationship between changes in motor conduction over time and clinical and MRI changes. Central motor conduction time (CMCT), serial MRI of the brain and spinal cord, and clinical evaluations were carried out in 20 pa...
متن کاملSustained-release oral fampridine in multiple sclerosis: a randomised, double-blind, controlled trial.
BACKGROUND Clinical studies suggested that fampridine (4-aminopyridine) improves motor function in people with multiple sclerosis. This phase III study assessed efficacy and safety of oral, sustained-release fampridine in people with ambulatory deficits due to multiple sclerosis. METHODS We undertook a randomised, multicentre, double-blind, controlled phase III trial. We randomly assigned 301...
متن کاملEffect of cooling suit treatment in patients with multiple sclerosis evaluated by evoked potentials.
The aim of the present study was to determine whether any significant alterations of evoked potentials could be detected after treatment of patients with multiple sclerosis with a cooling suit. All patients had previously experienced a positive effect of this treatment. Six patients were investigated with visual, sensory and motor evoked potentials and six further patients with only motor evoke...
متن کاملThe Effect of Oral Verapamil on the Visual Evoked Potentials in Definite Multiple Sclerosis Patients
Introduction: Ionic channel rearrangements through the demyelinated axons or supporting media play significant role in remission of the neurological deficit in MS patients. In this study the effects of Verapamil as a calcium channel blocker on central conduction have been investigated through the evaluation of changes in P100 latency of the visual evoked potential. Method: This randomized doubl...
متن کاملQuantitative assessment of sustained-release 4-aminopyridine for symptomatic treatment of multiple sclerosis.
OBJECTIVE To evaluate the efficacy of 4-aminopyridine sustained release (4AP SR) (fampridine, EL-970) using quantitative measures of motor function in multiple sclerosis (MS) patients. BACKGROUND In vitro, 4AP improves conduction through demyelinated axons. A previous multicenter trial of 4AP SR using the Expanded Disability Status Scale (EDSS) as the primary outcome was unable to establish c...
